RESPIRATION AND THE AIRWAY Emergency treatment of status asthmaticus with enoximone

چکیده
Editor’s key points † Enoximone is a phospodiesterase III inhibitor, primarily used in treatment of heart failure. † The author presents a case report of eight patients with status asthmaticus refractory to treatment. † Enoximone was effective in all these patients in reversing bronchospasm. † Enoximone may have a role in refractory status asthmaticus in preventing tracheal intubation and mechanical ventilation. This report describes the treatment of eight patients with status asthmaticus, six of whom were already maximally treated. They were consequently treated with enoximone, a selective phosphodiesterase III inhibitor, in their refractory phase. Bronchodilatation in these patients was immediate. No side-effects were observed. Enoximone appears to be a valuable addition to the treatment of status asthmaticus. I.V. administration bypasses inhalation incapability in severe asthma. It is likely to reduce or altogether prevent the need for resorting to secondary or tertiary high-tech therapy such as mechanical ventilation or anaesthetics, thus avoiding complications, as well as for transfers to specialized intensive care units. Not only do these aspects enable substantial cost savings, but they also may spare the patient a lot of anguish and a prolonged recovery.

Effective Approach to Potassium Permanganate Poisoning: Case Report and Review of Literature
Background: Potassium permanganate (PP) is a highly corrosive and deadly agent with a reported lethal dose of 10 g in adults. In this report, successful treatment of a patient poisoned with three times the lethal dose of PP is presented, and effective and early approach to such emergency toxicologic condition is discussed. Case Presentation:A 24-year-old woman presented to emergency room of our...
متن کاملThe role of chlorpromazine in the treatment of bronchial asthma and chronic pulmonary emphysema.
It is often difficult to achieve adequate sedation without concomitant respiratory depression in patients with bronchial asthma and chronic pulmonary emphysema. The deleterious and occasionally fatal effects of morphine in these disease states are well known.14 This agent is a primary and continuous depressant of respiration even with doses too small to produce sleep or disturb consciousness.5 ...

Trauma is the most common cause of mortality and morbidity in pediatric population. Caring for the injured child requires special knowledge, precise management, and scrupulous attention to details. All clinicians who are responsible for the care of a pediatric trauma patient, including pediatricians, pediatric emergency room clinicians, and trauma surgeons, must be familiar with every tenet of ...
متن کاملAsphyxia by tracheobronchial thrombus.
Asphyxia secondary to airway obstruction has numerous underlying causes, both acute and chronic. Causes of chronic airway obstruction, such as neoplasms and tracheal scarring, are often clinically apparent well prior to asphyxia. Causes of acute airway obstruction may not be as obvious to clinicians or investigators. These include infections, anaphylactic reactions, status asthmaticus, inhalati...
متن کاملPlasma exchange: an option for the treatment of life-threatening status asthmaticus in pregnancy.
Status asthmaticus during pregnancy is a rare but lifethreatening condition for the mother and foetus. Some authors favour the termination of pregnancy by caesarean section, while others have succeeded in saving the life of both the mother and the baby by the use of artificial ventilation in addition to conventional drug therapy. We report our experience with a rather unusual approach for the t...
